The Forms Module has the ability to print fifteen custom forms that the user can design. To help label these forms for easy identification, the list below is used to create either a pop-up list to select from. Form numbers 1, 11 and 12 have locked descriptions because they are "hard wired" in the system. The descriptions can be changed here or on the main menu of the Forms Module.
The description should be whatever if required to describe your custom form.
